ABOUT THE JOB

CONTRACT TYPE: Full-Time Permanent Contract.
WORKING HOURS: *.5 hours per week,
Monday-Sunday availability.
LOCATION: Dublin 7, next to Luas stop.
SALARY: *, * progressing

To implement and carry out effective care and case management systems
with the objective of progression to and sustainment of long-term
accommodation.

RESPONSIBILITIES

* Deliver a quality effective service, ensuring the progression and
sustainability of all residents.
* Manage a caseload as part of a diverse and motivated team working
to achieve service targets and individual goals.
* Work in collaboration with clients to create support plans,
inclusive of referral to services and accommodation move-on options,
where appropriate.
* Implement and work within care and case management systems and
protocols.
* Work in partnership with other agencies, both voluntary and
statutory, ensuring good communication and cross-agency working
practices to minimise obstacles to a person getting access to
supports, services and/or accommodation.
* Carry out ongoing assessment and reviews in order to provide a
quality evidence-based service, facilitating movement towards suitable
living and improved quality of life, in accordance with Simon
Community’s policies and procedures.
* Crisis and conflict management of risks to try and ensure the
protection of all staff and residents.
* Ensure residents are consulted and involved in the operation of
the project as far as possible.
* Provide care interventions, including daily living skills support,
relapse prevention support.
* Therapeutic and diversionary interventions, providing emotional
and motivational support, promoting positive mental health attitudes,
self-esteem, and mutual respect.
* Carry out front-line duties such as cleaning and room turnover, to
ensure the smooth running of the service.
* Maintain knowledge and awareness of mental and physical health and
addiction issues and services, with good practice and knowledge to
best support residents.
* Engage with and build a professional trusting relationship with
residents and staff, treating all with dignity and respect, in line
with the values of the Dublin Simon Community.
* Thorough record-keeping, consistent handovers, preparation of
reports, statistics and other information as required in order to
inform future strategy and responses.
* Work towards the achievement of individual and service goals as
determined by the local scorecard and organisational strategy.
* Participate in regular supervision and performance review,
contributing to the identification of own job related targets,
progression and training needs.

ESSENTIAL FOR THE ROLE:

* BA/BS degree (level 7 or 8) in Psychology, Community Development,
Counselling, Social Care, Social Work, or similar.
* 2+ years experience working in a social or caring setting.
* Understanding of why people become homeless and the needs they
have.
* Keyworking and advocacy skills, carrying out assessments, care
planning and review for individuals with a focus on outcomes.
* Commitment to promoting choice, independence, health, and
wellbeing.
* Ability to contribute to the prevention and management of
challenging behaviour.
* Commitment to Health & Safety
* Recordkeeping, report writing.
* Client-focused, adaptable, accountable, resilient individual.
* Excellent communicator, with the ability to advocate on the
client’s behalf.

DESIRABLE FOR THE ROLE:

* Knowledge of mental health diagnoses, treatment services and
mental health interventions.
* Understanding of addictions, trauma, behaviours and other
presenting issues.
* Full driver’s license
* Passion for helping people, with the ability to work in an agile
environment.
* Flexibility and openness to change.
* Planning, organising and adept at problem-solving.
* Effective team working and developing external relationships
* Resilience and positive outlook
